WELCOME TO HiHiP's LMT RENEWAL HELPLINE! This is a short guide on how to renew your DOH Massage Therapist License. YOUR MASSAGE THERAPIST LICENSE is valid from the date of last issuance until your birth month three years later. That means that your license expires and you should apply for renewal on your birth month every three years. The expiration date is indicated on your Certificate of Registration and your ID Card. WHERE SHOULD YOU APPLY FOR RENEWAL OF LICENSE? File your renewal application at the DOH Regional Office nearest you. Look for the Regional Licensing and Enforcement Division (RLED). The DOH Regional Director is the authenticating officer for license renewal. WHAT ARE THE REQUIREMENTS FOR RENEWAL OF LICENSE? The requirements are checklisted in the COVER SHEET provided by the RLED. You can download the cover sheet, print it on A4, and place it as the topmost page of your supporting documents. There are eleven (11) supporting documents you need to submit to the RLED.   SUPPORTING DOCUMENTS NOTES Medical Certificate Obtain this from a government physician or from the local health office "for license renewal purposes". Original is required. Professional Tax Receipt Licensed professionals must pay a professional tax at the start of every year in the LGU (City or Municipal Hall Cashier). You will be issued a Professional Tax Receipt or "PTR". NOTE: Some LGUs issue only an Occupational Tax Receipt or "OTR". Residence Certificate Obtain this from your LGU (Barangay or City Hall). Professional ID Card You need to surrender your ORIGINAL expiring ID card. In case of loss, submit a notarized affidavit of loss. Your latest Certificate of Registration can serve as backup. CMTE/CEE Certification of Credit Units Earned Together with your Certificate of Training, this is a signed and notarized certification that the 30 continuing education units earned -- are true and valid. It is designated as CEMT FORM 3. Request three (3) original copies from your DOH Accredited Training Provider (for notary, for RLED, and for yourself). Certificate of Registration OR the certificate of last renewal. ID picture 1x1 Two (2) pieces recent ID photo. ID picture 2x2 One (1) piece, with your full name below it. Passport size is acceptable. Category 1 -- Practicing If you are an employee, submit an Employer's Certificate.   If you are a freelancer, submit a certificate of work from the barangay where you work.   If you are a spa owner, submit a copy of your business permit. Category 2 -- Non-Practicing If you are not actively practicing massage therapy, submit a notarized letter that you are not practicing the profession at the moment, but that you still want to be included in the registry of massage therapy. Barangay Certificate of Good Moral Character Submit an updated certificate of good moral character from your barangay. Renewal Fee Receipt Obtain the Official Receipt when you pay the renewal fee in the Regional Office Cashier. It is advisable to photocopy and/or take a photo of this Official Receipt. If your license has expired, you will be charged a fine for late renewal. WHERE IS THE DOH REGIONAL OFFICE FOR CALABARZON (REGION IV-A)? CONTINUING MASSAGE THERAPY EDUCATION SEMINARS

REGULATED PROFESSIONALS are mandated by law to undergo continuing education. For Licensed Massage Therapists, a minimum of 30 credit units is required for license renewal every three years. According to the Code on Sanitation of the Philippines (PD No. 856), massage practitioners are regulated by the Department of Health, which operates through its Committee of Examiners for Massage Therapy (CEMT). WHAT SEMINARS CAN I ENROLL IN? You are free to enroll in any seminar that is approved by the CEMT's Continuing Massage Therapy Education Council (CMTEC). If you attend seminars in the PALMT National Convention, you can also gain 30 credit units. HiHiP offers a two part course that is valid for license renewal.

HOW DO YOU RESERVE A SLOT FOR THE HiHiP SEMINAR? You must sign up to reserve your slot. Just follow three easy steps. FILL OUT the form at License Renewal Help Online; MESSAGE your Name, License Number, CP#, and Location to Parthia Hihip on Facebook; and TEXT the same information to HiHiP CP# 0906 222 0064. WHERE WILL THE SEMINAR BE HELD?  The training venue will be chosen by the participants, subject to approval by HiHiP. It can be a classroom, conference room, or spa. HiHiP will provide the training -- anywhere in the Philippines -- provided there are at least ten (10) participants enrolled in the area. WHEN WILL THE SEMINAR BE HELD? As soon as there are ten LMTs who have signed up, HiHiP will set the seminar dates, after due consultation with the group members. HOW DO YOU ENROLL FOR THE HiHiP SEMINAR? After reserving a slot and confirming the seminar date and venue, just pay the seminar fee in any branch of Asia United Bank. Then, post your AUB transaction slip on the renewal chat group on Facebook. Make sure you write your name, the purpose of your payment, and the training venue on the slip before posting it.
